HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can access this information.

Who We Are

Aware Physical Therapy Prof. Corp, a California professional corporation (“PC”), provides a variety of health and wellness services through its employed clinicians and health coaches.

Administrative, operational, and technology services are provided by Aware Health Labs, Inc., our affiliated management services organization (“MSO”), which supports the PC.

We may provide these services to you on behalf of your health plan or employer, as further described in Aware Health's Terms of Service ("Services").

In this notice, we refer to the PC and MSO together as Aware Health ("Aware").

Our Responsibilities

We are required by law to:

• Maintain the privacy and security of your Protected Health Information (PHI) through reasonable administrative, physical, and technical means
• Provide you with this Notice
• Follow the privacy practices described in this Notice
• Let you know if a breach occurs that may have compromised the privacy or security of your PHI

We may update this Notice from time to time, and will make the updated version available.

How We Use and Disclose Your Information

The following categories describe different ways we may use and disclose your PHI (without your authorization), as permitted by law. Not every permitted use or disclosure is listed, but all permitted uses and disclosures fall within one of these categories.

Treatment: To provide, coordinate, and manage your healthcare and related services.

Payment: To bill and receive payment from health plans or other entities.

Healthcare Operations: To operate our business and improve care, including but not limited to care coordination and administration, quality assessment and improvement, training and education, and other business activities.

Organizational Structure and Disclosures

Your care is provided by clinicians employed by our PC.

Our affiliated MSO provides administrative and operational support and may access PHI as a business associate to perform these services.

We may share your PHI:

• Between the PC and MSO for treatment, payment, and healthcare operations
• With your health plan or employer as permitted or required for payment and care coordination

Other Uses and Disclosures

We may also use or disclose your PHI as permitted or required by law, including:

• Public health and safety activities
• Health oversight activities
• Law enforcement requests
• Research (subject to applicable requirements)
• To prevent or lessen a serious threat to health or safety

We will obtain your written authorization for uses and disclosures not described in this Notice as required by law, and you may revoke such authorization at any time in writing.

Your Rights

You have various rights when it comes to your PHI, including:

• Access and receive a copy of your records
• Request corrections to your information
• Request restrictions on certain uses or disclosures
• Request confidential communications
• Receive an accounting of disclosures
• Obtain a copy of this Notice
